Van Cleef's screen career started with a refusal. Cast in High Noon — his debut — he turned down a studio request to surgically alter his nose if he wanted a sympathetic role, and wound up playing a silent outlaw instead (Wikipedia). Not the most auspicious start. But the face that Hollywood found inconvenient for heroes turned out to be exactly what audiences wanted from villains, and for the better part of a decade he worked steadily as a supporting bad guy, logging dozens of television and film appearances that rarely gave him room to do much beyond glower convincingly. The real turning point came in 1965, when Sergio Leone cast him in For a Few Dollars More. Van Cleef had reportedly suffered serious injuries in a car crash before this period and was losing interest in a career that seemed to be stalling (TMDB). Leone didn't see a washed-up character actor — he saw a face that could anchor a film. The gamble paid off. For a Few Dollars More made Van Cleef a genuine draw, and The Good, the Bad and the Ugly cemented it. He was playing leads now, not footnotes. The thing nobody mentions is how rare that kind of second act actually is in Hollywood — most actors who spend ten years in the margins don't get a Leone to pull them out. Through the late 1960s and 1970s, he built on that momentum with a string of Italian and international westerns and action pictures, the kind of genre work that doesn't win awards but does build a loyal audience. He wasn't locked into the western format either — his career showed enough flexibility to keep him working across action cinema more broadly. Despite being diagnosed with heart disease in the late 1970s and having a pacemaker fitted in the early 1980s, Van Cleef didn't slow down (TMDB). He continued taking roles in film and television right up until his death on December 16, 1989. A career measured not in Oscars but in sheer, stubborn output.
